NACCHO actively supports local approaches to chest/breastfeeding protection, promotion, and support. NACCHO is currently implementing the Reducing Breastfeeding Disparities through Continuity of Care project to develop a national Continuity of Care (COC) blueprint with national partners, provide targeted technical assistance to CDC REACH grantees in COC, and funding Continuity of Care community partnership activities. This project also includes identifying best practices to improve the infant and toddler nutrition landscape within the first 1,000 days of life.
Through webinars, ongoing technical assistance, and success stories, NACCHO shared lessons learned and effective strategies for local level implementation. NACCHO continues to provide training and technical assistance to local health departments and other community-based organizations to advance community support and continuity of care.

The Continuity of Care in Breastfeeding Support: A Blueprint for Communities
The Blueprint, developed with a public health lens, aims to increase local capacity to implement community-driven approaches to support chest/breastfeeding, centered on the needs of populations disproportionately impacted by structural barriers that lead to low rates of breastfeeding. The goal of this resource is to ensure that chest/breastfeeding support services are continuous, accessible, and coordinated, and that community spaces are consistently supportive of chest/breastfeeding families. The blueprint is intended for any local-level organization and individual that interacts with pregnant and postpartum families.
Community Partners Implementing the Blueprint: Continuity of Care in Breastfeeding Support Cohort II
With support from the Centers for Disease Control and Prevention, Division of Nutrition, Physical Activity, and Obesity (CDC/DNPAO), NACCHO is pleased to announce a Request for Applications to strengthen community lactation support, through the implementation of the Continuity of Care in Breastfeeding Support: A Blueprint for Communities. The purpose of this funding opportunity is to support the implementation of Continuity of Care (CoC) strategies by local-level organizations among oppressed communities with historically low rates of chest/breastfeeding. Applicants are required to apply with at least two other key partners to implement strategies from at least four (4) Blueprint recommendations.
Implementation Awards: Up to 5 local organizations will be awarded up to $40,000 each

Implementing the Continuity of Care in Breastfeeding Support: A Blueprint for Communities
From November 2021 to July 2022, NACCHO awarded ten (10) organizations $20,000 to implement at least two strategies from the Continuity of Care in Breastfeeding Support: A Blueprint for Communities to improve the local chest/breastfeeding landscape in their communities. Selected awardees have started implementing Blueprint recommendations with at least one community partner. The selected organizations are the following: Asian, Southeast Asian, and Pacific Islander (ASAP) Breastfeeding Taskforce (CA), Black Mother's Breastfeeding Association (MI), Cincinnati Children's Hospital Medical Center (OH), Florida Keys Healthy Start Program (FL), Kansas Breastfeeding Coalition (KS), Michigan Breastfeeding Network (MI), Minnesota Breastfeeding Coalition (MN), Queen City Cocoa B.E.A.N.S (NC), Thames Valley Council for Community Action Inc. Identifying Care Gaps: Conducting Community Assessments to Improve the Chest/Breastfeeding Landscape in Historically Oppressed Communities
From March 2021 to November 2021, NACCHO awarded eight (8) organizations to conduct a community assessment of the lactation landscape at the local level with the intent to utilize findings to improve chest/breastfeeding rates in oppressed communities with historically low rates of breastfeeding. Selected awardees implemented their project activities with key community partners. The project ended in November 2021. The selected organizations are the following: the Lactancia Latina of Southwest Kansas (KS), Hmong Breastfeeding Coalition (MN), Northeast Valley Health Corporation (CA), Healthy Hearts Plus II (VA), East Side Health District (MO), Coahoma County Diaper Bank (MS), Children's Hospital of Pennsylvania- Division of Neonatology (PA), and Center for African American Health (CO). All resources and toolkits from this project are on the Continuity of Care in Breastfeeding Support Blueprint resource library under Recommendation 1 (Community Assessments).

This report outlines the critical role of Community Health Centers (CHCs) in promoting, protecting, and supporting breastfeeding in their communities. It includes a compilation of NACCHO's previous CHC grantees tools and stories of successful implementations of organizational policies, systems, and environmental (PSE) changes to establish internal breastfeeding support continuity of care. It covers a broad range of practical information that will support communities to ensure evidence-informed practices for successful sustainable programs!

The Breastfeeding team published two open-access peer-reviewed articles focused on social justice and lactation in the Spring 2018 issue of the Journal of Human Lactation. They were the 2019 top 2 most read articles of the Journal of Human Lactation!
"Breastfeeding in the Community: Sharing Stories on Implementations That Work" highlights the work of 19 NACCHO breastfeeding project grantees.
In " Breastfeeding in the Community: Addressing Disparities Through Policy, Systems, and Environmental Changes Interventions," the authors share practice-oriented strategies for agencies seeking to implement community-level breastfeeding interventions through a public health policy, systems, and environmental change approach.
